Written by a Master's prepared nurse practitioner with extensive palliative care background, these palliative care policies can be used by an CHAP organization as the backbone of their palliative care program. The policies are clinical in scope due to the variety of palliative care program models being utilized. Mission, Scope of Services, Admission Criteria and Process, Plan of Care, Case Management, Discharge and Patient Education are just a few of the palliative care policies included.
A total of 23 policies, 6 job descriptions, a Palliative Care competency checklist and a Crosswalk of CHAP Standards are included. These policies were vetted and approved by CHAP.
